When Should Eyelid Surgery Be Considered?

What to Expect During Your Blepharoplasty

Most of our patients’ eyelid surgeries are performed on an outpatient basis, typically taking between thirty to forty minutes for upper eyelid surgery and about one hour for combined upper and lower eyelid surgery.

Drs. Ron and Alex Caniglia perform all eyelid procedures under local anesthesia with intravenous sedation, also known as deep twilight sleep. This form of anesthesia keeps the patient from feeling any discomfort or remembering anything from surgery while also allowing them to feel more alert and refreshed after surgery.

Upper Eyelid Blepharoplasty

  1. Anesthesia: The procedure starts with the administration of local anesthesia with intravenous sedation (deep twilight sleep) to make sure you are comfortable and pain-free throughout the surgery.
  2. Incision: A small incision is made in the natural crease of the upper eyelid. This placement ensures that any resulting scar will be well-hidden.
  3. Removal of Excess Tissue: The surgeon removes excess skin and, if necessary, recontours the underlying fatty tissue to eliminate puffiness. A thin strip of muscle may also be removed to achieve the desired look.
  4. Closure: The incision is closed with a very fine suture hidden underneath the skin.

Lower Eyelid Blepharoplasty

There are three different surgical approaches to lower eyelid surgery, determined by the specific needs of the patient:

Transconjunctival Approach

  1. Anesthesia: Local anesthesia with intravenous sedation is administered.
  2. Incision: A laser is used to make a precise incision inside the lower eyelid, ensuring no visible scars.
  3. Recontouring Fat Pads: The three fat pads in the lower eyelids are recontoured to eliminate puffiness.
  4. Closure: The incision inside the eyelid closes on its own and does not require stitches.
  5. The excess skin of the lower eyelids is treated in one of two manners. A small strip of skin is removed below the eyelashes to tighten the lower eyelid skin. A second approach involves using the CO2 laser, which both tightens the skin and helps to smooth wrinkles and crepiness.

Skin-Muscle Flap Approach

  1. Anesthesia: Local anesthesia with intravenous sedation is administered.
  2. Incision: An incision is made along the lower lash line.
  3. Recontouring and Tightening: Excess skin is removed, and the lower eyelid muscle is tightened to improve tone and appearance.
  4. Closure: Dr. Caniglia closes the incision with fine sutures.

Skin Flap Only Approach

  1. Anesthesia: Local anesthesia with intravenous sedation is administered.
  2. Incision: A small incision is made just below the lower lashes.
  3. Removal of Excess Skin: Excess skin is removed to smooth and tighten the lower eyelid.
  4. Closure: The incision is closed with fine sutures.

Blepharoplasty Recovery

Eyelid surgery recovery is simple and quick. There is very little to no discomfort the next day. Bruising is minimal, particularly in the lower eyelids, when using the laser and inside approach. All sutures are removed, and patients can return to work in seven days. Drs. Ron and Alex Caniglia ask all patients to wait two weeks before resuming heavy, strenuous activity or exercise. There is very little pain with eyelid surgery.

The eyes are not swollen shut, and you will be able to see that day. Sutures are removed in seven days. Women can apply makeup after suture removal. Normal activity can be resumed after ten days. If laser skin resurfacing of the lower eyelid skin is combined with the lower eyelid surgery, women will need 10-14 days before they can apply camouflage makeup.
