How is a Rhinoplasty Procedure Done?

Most rhinoplasty surgeries are performed outpatient and typically take one to two hours to complete. Surgical times may be longer for extensive reconstructive rhinoplasties if multiple grafts are needed.

Drs. Ron and Alex Caniglia perform all rhinoplasty procedures under local anesthesia with intravenous sedation (deep twilight sleep administered by a board-certified anesthesiologist), ensuring patients feel no discomfort and have no memory of the surgery. Patients feel alert and refreshed following the procedure, and a board-certified anesthesiologist monitors them before, during, and after surgery.

There are two approaches to rhinoplasty surgery: the closed approach and the open approach. Both use incisions hidden inside the nose. The open approach includes an additional small inverted “V” incision at the base of the nose, which leaves a nearly imperceptible scar.

Either procedure typically involves the following steps:

  1. The skin overlying the nasal tip and dorsum (hump) is elevated by working through mostly internal incisions.
  2. The tip cartilages are reshaped, either reducing or enlarging the size of the tip. Depending on whether the nose needs to be shortened or lengthened, the tip can also be turned up or down.
  3. The shape of the nostrils is addressed, either narrowing or reducing the flare as needed.
  4. The bridge of the nose is often lowered or, in some cases, raised.
  5. The width of the bridge is evaluated and narrowed, if necessary, through controlled incisions in the nasal bones performed inside the nose.
  6. Once the tip and bridge of the nose are in good aesthetic harmony, the incisions are closed with tiny sutures, and a thin splint is applied externally to the nose. Internal packing is usually unnecessary, making recovery virtually painless.

What Happens After the Rhinoplasty Procedure?

There is typically very little discomfort following rhinoplasty surgery, especially when internal packing is avoided. Thanks to the expertise and proper surgical techniques of Drs. The external splint is also removed on postoperative day seven, and many patients return to work once the splint is removed. Patients are advised to wait two weeks before resuming heavy, strenuous activity or exercise.
